Thank you for choosing Accurate Lab Testing. Our goal is to provide accurate, professional, and compliant testing services in accordance with all applicable federal, state, and industry regulations. Please review the following policies prior to scheduling your appointment.

 

Payment Policy

​Payment is due at the time of service unless prior arrangements have been made with an employer or authorized organization. Accurate Lab Testing accepts major debit and credit cards through our secure payment system.

 

Prices listed for services reflect in-office collections during normal business hours. Additional fees may apply for mobile services, after-hours appointments, or specialized testing requirements.

 

Appointment & Cancellation Policy

We understand that schedules may change. If you need to reschedule your appointment, please contact us as soon as possible.

Clients who fail to appear for their scheduled appointment without notice may be subject to a missed appointment fee.

 

Identification Requirements

All individuals being tested must present a valid government-issued photo identification at the time of service. Acceptable forms of identification include:

 

• Driver’s license

• State identification card

• Passport

• Military identification

 

Testing cannot be performed without proper identification.

 

Compliance & Collection Procedures

Accurate Lab Testing strictly follows U.S. Department of Transportation (DOT) regulations and federal collection guidelines where applicable. All collections are conducted according to established chain-of-custody procedures to ensure accuracy, integrity, and compliance.

 

Any attempt to tamper with, substitute, or otherwise compromise the specimen collection process will result in immediate termination of the test and proper documentation in accordance with regulatory guidelines.

 

Refund Policy

Due to the nature of laboratory and specimen collection services, all sales are final once the collection process has been completed.

 

Refunds will not be issued for completed services. If a test must be recollected due to circumstances outside of the client’s control, Accurate Lab Testing will provide further instructions based on the requirements of the testing program.

Mobile Service Policy

Mobile testing services are available for certain tests and may include a travel or convenience fee. Mobile appointments are subject to availability and may require advance scheduling.

Additional travel fees may apply depending on the service location.

 

Privacy & Confidentiality

Accurate Lab Testing respects the privacy of all clients. Personal and testing information is handled with strict confidentiality and in accordance with applicable privacy and data protection standards.

 

Test results are only released to authorized parties as permitted by law or by written authorization.
